Our wrap around services offer a continuum of care with collaborative case management and in-home mentoring that bridges the gap between individuals, families, and treatment providers while facilitating a successful healing process. Such support allows individuals and their families to move out of crisis and into a solution.

TYPES OF THERAPEUTIC MENTORING

ADDICTION


Mentors support individuals struggling with addiction by creating an individualized self-care plan to create structure, accountability, and consistency.

Our mentors support individuals struggling with an addiction by creating an individualized self-care plan to establish structure, accountability and consistency. Such action plans are broken down into daily steps that provide a roadmap leading them into a life of recovery. Mentors will facilitate individuals developing a strong engagement in a 12 step or recovery program that will allow them to develop healthy relationships and expand their support system in their community. Mentors will also assist in the development of relapse prevention plans for daily routines, family and social engagements and emotional triggers. Bridge The Gap Services offer drug testing and breathalyzing services upon request.

 
 
 

MENTAL HEALTH


Mentorship acts as critical support to an individual coping with symptoms of a mental health diagnosis to reach their therapeutic goals and gain relief.

MENTAL HEALTH MENTORING


Mental health struggles can feel overwhelming, lonely and often times discouraging. The reality is that the majority of the population faces mental health challenges at some point during their lives. You do not have to face these feelings alone. Mentorship acts as a critical support to an individual coping with symptoms of a mental health diagnosis. Mentors work alongside an individual’s treatment team (therapist, psychiatrist, nutritionist Etc.) to reach their therapeutic goals, utilize healthy coping strategies and gain symptom relief. A mentor may help a client face and successfully move through anxiety provoking engagements, debilitating phobias, or taking contrary action to mood dependent behaviors. To do so, mentors will collaborate with the individual and their support system to execute an individualized self-care plan. Once this action plan is established, the mentor will support the client in follow through, structure and consistency. When necessary, mentors may also support their clients with medication management.

Mentors support individuals in this life transition by modeling, teaching, and facilitating the execution of learning responsibility and being able to confidently be in the world in a productive and empowering way.

The idea of being an adult can be daunting and overwhelming. Where do you start? What does that look like? Mentors support individuals in this life transition by modeling, teaching and facilitating the execution of learning responsibility and being able to confidently be in the world in a productive and empowering way. Individuals will be given a calendar to develop a consistent and structured daily action plan. Once a plan is in place, mentors will support the launcher with developing a resume, securing a job, budgeting, developing a self-care plan and discovering their passions to be able to take steps to reach their future goals. As the individual begins to reach their goals, find their purpose and maintain consistency, they will develop self-esteem and empowerment that is imperative in them successfully launching into adulthood.

TRANSITIONS


Transitioning from a higher level of care to a lower level of care or home environment can be challenging to navigate alone. A mentor will facilitate this transition to be smooth, successful, and empowering.

A mentor will facilitate this transition to be smooth, successful and empowering. Mentors support individuals in utilizing a calendar to develop a consistent and structured daily action plan that allows them to maintain productivity, follow through with commitments, and execute an individualized self-care plan developed upon engagement in mentorship. A mentor will provide assistance in reintegrating back into their home environment and community by supporting them to confront emotional triggers, day to day challenges, family dynamics, conflict and stressors by utilizing healthy coping strategies and developing a strong support system. As the individual reestablishes a safe environment and healthy routine, they will gain a sense of empowerment, esteem and confidence to maintain this on their own.

 
 
 

STUDENTS


Mentors act as an in person guides to support teens and their families in finding a healthy balance between school, friends, self-care, and family time.

STUDENT MENTORING


Academics, peer pressures, friendships and hormones can be extremely challenging to navigate for teens and their parents. Although there are informative and informational guides to moving through the teenage years successfully, reading versus applying these suggestions are entirely different actions. Mentors act as in-person guides to support teens and their families in finding a healthy balance between school, friends, self-care and family time. To do this, mentors provide each individual with a planner to visually organize and prioritize their schedules while following through with their responsibilities and reaching their goals. If an individual is struggling with focus, completing homework or taking tests, mentors will supply them with helpful tools and necessary resources to work through these challenges. For teens struggling with peer pressures and friendships, mentors will act as a daily support to help them process challenging emotions and stressors while developing and utilizing healthy coping strategies. A mentor that is relatable and acts as ‘their person’ can provide teens with a safe person to confide in and process the many challenges of being a teenager in today’s world. Mentors will support teens and their families in setting and maintaining healthy boundaries necessary to maintaining a healthy and communicative home environment.

 
 

What is Therapeutic Mentoring? 


Individuals and families often seek a therapeutic mentor when they or a loved one is in need of support beyond the therapy room to implement their therapeutic skills and reach their goals. Getting support beyond the therapy room or treatment environment provides individuals with accountability and guidance in developing goals, breaking them down into reachable steps, and gaining empowerment to transition into doing this on their own. A therapeutic mentor will also relieve family members of their management role to be able to work on their familial relationships and family dynamics. 

The therapeutic world can be daunting to navigate. Therapeutic mentors will facilitate this process by connecting everyone involved on an individual’s support team by providing them with weekly updates to ensure collaborative, comprehensive, and optimal care.
