When in-office therapy alone isn’t enough, Bridge The Gap Services is there to offer hands-on support when and where your family needs it most: through the ups and downs of everyday life.
Whether your child is transitioning from a treatment center back into the home, or needing support to reach their career and/or educational goals, our Bridge The Gap Program is created to offer the specialized treatment they need to thrive.
Transition-to-Home Package
This specialized aftercare program was designed to create a comprehensive family support system around your child as they transition from a treatment facility back to home recovery.

We initiate the process while your child is still in residential treatment by connecting with their treatment center team. This allows us to gain insight into their treatment progress, recovery goals achieved, and areas needing further attention.

If a family contract or home agreement isn’t already in place, we assist in creating one to ensure everyone is on the same page and that clear boundaries and healthy expectations are set before your child’s return home.

Prior to discharge from treatment, we facilitate a meet-and-greet session between the client and their recovery mentor via Zoom. This starts building rapport and trust early, helping to smooth the transition to home.

Once home from treatment, clients will have defined recovery goals and a clear understanding of how their mentor support will assist them. Based on the treatment team’s recommendations, clients will receive a minimum of four hours per week with their mentor—typically more initially, tapering off as they achieve greater stability in recovery.

This package typically spans 3 to 6 months, providing ongoing support as clients adapt to their new environment.
This package ensures a structured and supportive transition, enhancing the likelihood of sustained success after treatment.

Young Adult Launch Package
Whether your teen is struggling to establish independence, find a job after treatment, or transition to college life, the Young Adult Launch Package offers high-touch transitional support to help your child thrive in school, at work, and at home.

We collaborate with educators, therapists, and family support teams to gain a holistic understanding of the young adult's mental health challenges, educational needs, and long-term life goals.

Your teen is matched with a dedicated recovery mentor who offers individualized support based on their specific circumstances, fostering a strong, trusting relationship that empowers them to navigate life transitions, build self-confidence, and develop life skills for young adults.

Mentors assist in organizing the essentials for independent living skills, including daily routines, budgeting, and time management, ensuring they feel equipped and comfortable in their new environment.

Our mentorship includes assistance with course registration, academic coaching, creating personalized study plans, and tackling any academic challenges, providing the tools necessary for student success.

Young adults learn to effectively utilize resources like office hours and develop meaningful connections with their co-workers, professors, and peers, promoting a supportive network.

Mentors help create manageable weekly schedules that balance academic responsibilities, job commitments, and personal time, fostering effective time management skills.

Meetings can be held virtually or in-person if local, ensuring accessibility and convenience.

Executive Functioning Package
The Executive Functioning Mentorship Package is designed to help clients enhance their organizational and time management skills, leading to greater success at school and at work.

We begin by collaborating with all treatment teams, school counselors and teachers, as well as parents to identify specific areas of executive functioning that may be causing challenges, whether related to school for teens or work-related issues for adults.

After the assessment, clients are paired with a mentor through a meet-and-greet to ensure a good fit. Together, they set both short-term and long-term goals, such as achieving a specific GPA or securing employment.

Long-term goals provide direction, while short-term goals focus on manageable, week-to-week tasks like homework and to-do lists, making the overall process less overwhelming.

Mentors work closely with clients to develop effective time management strategies, breaking tasks into smaller, achievable steps.

Weekly check-ins provide accountability, with mentors prompting clients on daily tasks, such as “Did you make your bed today?” to build consistent habits.

Sobriety Coaching Package
Our experienced substance abuse mentors are here to support your teen or young adult in creating a sustainable program of recovery.
From working the 12-step program, to identifying triggers and implementing healthy coping tools, this comprehensive package meets each client where they are in their recovery journey.
